This is where you get to tell me how you want to Evviva-ify your life and find your vibrancy again.

Evviva is designed for women in their middling years. Pardon me for being so rude (gasp!), but what decade of life are you in? (So that I can put you into groups of similar life stage.) *

Have you ever been given a mental health diagnosis? (or diagnosed yourself) with any of the following: depression, anxiety, ADHD, ASD, active or previous suicidality, active or previous eating disorder, active or previous addictions, Borderline Personality Disorder, Schizophrenia. *

(These do not necessarily exclude you from Evviva. I need to make sure that you're in a good place so that group therapy is right for you. Sometimes 1:1 work is better if you're struggling a bit.)
